A long root that needs a larger apex

Long roots? Thirty-one millimetres is the length for a long canine, a tall premolar or any root where a 25 mm file leaves the rubber stop short of the reference cusp. This is the Medium – wider canals and larger apical preparations – in that length.

When the Primary reaches length loosely

Gauging is the step that decides this. If the Primary drops to working length without resistance, the apical preparation is smaller than the canal it is sitting in, and the file that fits is the Medium – a 0.35 mm tip on a .06 variable taper.

The taper steps down as the tip diameter goes up, which is the point of the range: a bigger apical size need not mean a bigger coronal one. On a distal or palatal root that is the difference between a shaped canal and a thinned one.

Necrotic cases and the irrigation argument

A larger apical preparation is the usual answer where disinfection is doing the heavy lifting: a necrotic tooth, a retreatment, a canal you want the irrigant to actually reach and exchange in. The Medium gives that without a separate instrument sequence.

It is still one file, one motion and one motor setting – the Medium follows the Primary in the same reciprocating mode, so nothing about the case set-up changes.

On the taper figure: .06 is the nominal designation at the tip. The taper eases coronally rather than holding .06 up the shaft, and Dentsply publish no average-taper figure for WaveOne® Gold, so none is quoted here.

Questions we get asked

› Do I still need a glide path? Yes. Secure the canal to length with a #10 hand file and confirm patency, then the Gold Glider creates the glide path in the same reciprocating motion. The instructions for use require a secured, patent canal first.

› Do I use the Medium on its own? No. It follows the Primary in the sequence – the Primary establishes the shape, gauging tells you whether to step up.

› What motor do I need? Any motor carrying the WaveOne setting – X-Smart® Pro+, X-Smart® Go, VDW.SILVER® and VDW.GOLD® RECIPROC®.
