Straight-line access costs dentine at the neck of the tooth, and that is the part TruNatomy® is designed to keep. The Orifice Modifier is the first rotary file in Dentsply Sirona’s sequence: it shapes the orifice to create the entry point for the Glider and shaping files, and works only in the coronal third. The Orifice Modifier comes in one length, 16 mm.

› Entry point, not a straight-line cavity. Dentsply Sirona: “TruNatomy® Orifice Modifier shapes the orifice to create the entry point without straight line access.” Their data on file.
› Coronal third only. Step 5 of the Solution Guide: “The instrument has 7 mm of cutting flutes, insertion into the canal should not exceed the length of the cutting portion.”
› One length. Dentsply Sirona list it at 16 mm only – its job never takes it deeper than its flutes.
1 Scout the coronal two-thirds. An ISO 010 K-file with a lubricant such as Glyde™ File Prep, then irrigate (step 4).
2 Orifice Modifier. With irrigant in the canal, 2–3 gentle amplitudes of about 2–5 mm, repeated until the coronal third is shaped. Irrigate and clean the flutes (step 5).
3 Then the whole canal. Scout to length with the ISO 010 K-file, working length by apex locator and radiograph, confirm patency (step 6) – then the Glider.

Do I still need to prepare an access cavity?
Yes. Step 3: “Prepare a conservative access cavity sufficient enough to reveal all root canal orifices.” The Orifice Modifier works from there.
Why is there no 21, 25 or 31 mm version?
Because it never goes past its 7 mm of flutes. Dentsply Sirona list it at 16 mm only.
Which motor settings?
Dentsply Sirona give one setting for every TruNatomy® file: 500 rpm and 1.5 Ncm. Set it on any of the three endo motors we stock – X-Smart® Go · X-Smart® Pro+ · VDW.SILVER® RECIPROC®.
When do I throw a file away?
Dentsply Sirona’s procedure (step 11): inspect the cutting flutes routinely on removal for unwinding and straightening, and if deformation is noted, discard it and use a new TruNatomy® instrument.
